Add html anchors to line numbers for "=sourcedcode" (RT#63526)
Now installs cleanly when Text::Aspell is missing (RT#63541) Removed Text::Aspell as a required module in Makefile.PL Tests SKIP if Text::Aspell is missing.
Support optional display text for M<> (RT#63525)
Add optional paramter so that "=begin pre "encode_entities" will auto-entify contents w/HTML::Entities::encode_entities(). (RT#63527) Added HTML::Entities as a dependency in Makefile.PL.
test suite tweaks to add a little more coverage (now at 98.6%)
added docs/coverage.txt to the distro.
